Your form has been submitted.

One of our team will contact your comrade to help them complete the full application form. 

There are number of ways we can help them register, which you may want to let them know about in advance.

• The quickest is via our online form
• They can do it over the phone with one of our team
• A printed application can be posted to them, or downloaded

Please note, once we come to fill in the application form with them we will need the following documents to confirm their service record and assess whether they meet our sight loss criteria:

• Service papers
• Eye test report
• Certificate of visual impairment (CVI)
